How to qualify medically for vein treatment
Qualifying for insurance-covered vein treatment is a documentation process, not a guessing game. Patients who follow the four-step path below are approved by virtually every major insurer.
Step 1 — Document symptoms
Insurers require evidence that vein disease is affecting your daily life. Symptoms include leg heaviness, aching, swelling, restless legs, skin discoloration, healing wounds, or recurrent superficial phlebitis. Your specialist will record these in the chart.
Step 2 — Get a duplex ultrasound
A duplex ultrasound is the diagnostic test that confirms venous reflux. The technologist measures reflux time at multiple segments. Reflux > 500 ms in the saphenous or tributary system establishes the diagnosis insurers require.
Step 3 — Complete a conservative therapy trial
Wear graduated compression stockings (20–30 mmHg or higher) for 6–12 weeks (depending on insurer). Continue elevation, walking, and any prescribed NSAIDs. Your specialist documents the trial in the chart.
Step 4 — Prior authorization
Your partner practice submits the symptoms, ultrasound, and conservative therapy documentation to your insurer for prior authorization. Approval typically arrives in 5–10 business days.
Frequently asked questions
Can I do conservative therapy on my own first?+
It's helpful, but the insurer needs the trial documented in a clinical chart by a physician — so most patients combine self-care with periodic specialist visits during the trial period.
What if my ultrasound shows reflux only in tributaries?+
Tributary reflux can still qualify for treatment, particularly when symptomatic varicose veins are visible. Your specialist will determine the right approach.
