Medicare coverage for varicose vein treatment
Medicare covers varicose vein treatment when symptoms, ultrasound findings, and a trial of conservative therapy are documented. Coverage rules are set by Local Coverage Determinations (LCDs) issued by Medicare Administrative Contractors and apply to both Original Medicare and most Medicare Advantage plans.
What Medicare requires
Medicare's published LCDs require all of the following before approving endovenous ablation:
- Documented symptoms interfering with daily activities
- Vein diameter ≥ 5.0 mm (varies slightly by region)
- Reflux > 500 ms on duplex ultrasound
- ≥ 6 weeks (sometimes 12) of conservative therapy with graduated compression
- Failure of conservative therapy to relieve symptoms
Procedures Medicare covers
Radiofrequency ablation (RFA), endovenous laser ablation (EVLA), microphlebectomy of bulging tributaries, and ultrasound-guided foam sclerotherapy of larger refluxing veins are all covered when criteria are met. VenaSeal coverage varies by MAC region.
Patient out-of-pocket under Medicare
After the Part B deductible, Medicare pays 80% of the approved amount. Most patients have a Medigap plan or Medicare Advantage plan that covers the remaining 20%, leaving little or no patient cost.
